Effect of phentolamine on atrioventricular conduction in man assessed by recording His bundle potential.

چکیده
The effect of phentolamine on atrioventricular conduction was studied in 6 patients using the technique of recording the His bundle potential. Right atrial pacing was used in order to keep the heart rate constant before and after administration of the drug. Increasing the heart rate by right atrial pacing resulted in a progressive increase in the PH interval, which represents atrial and atrioventricular conduction time, but had no effect on the HV interval, which represents intraventricular conduction. Ten mg phentolamine intravenously effectively shortened the PH interval and had no effect on the HV interval. In one patient this effect was blocked by the previous administration of 5 mg propranolol intravenously, suggesting that the mode of action of phentolamine may be through local release of catecholamines. Our results show that phentolamine improves atrioventricular nodal conduction and has no effect on intraventricular conduction.

Electrophysiological properties of phentolamine in man.
The effects of phentolamine, 0-3 mg/min given intravenously for 15 minutes, on His bundle electrograms were studied in 11 patients with heart disease. Recordings were made at varied heart rates, using atrial pacing. Phentolamine significantly reduced the AH interval in every patient but it had no effect on the HV interval. Functional and effective refractory periods were measured with the use o...
متن کاملCatheter recording and validation of left bundle-branch potentials in intact dogs.
The left bundle-branch potential (LB), as well as His bundle (H) and atrioventricular nodal (N) potentials, were recorded in intact dogs by using a tripolar electrode catheter positioned in the left ventricle. Simultaneous recordings of H and right bundlebranch (RB) activity from the right endocardial surface were also made. The LB potential was less rapid than the RB potential, but both occurr...

Since the recording of the first human electrocardiogram by Wailer in I889 the science of electrocardiography has developed and advanced rapidly, but has been limited by the inability to record electrical activity from specialized areas of the heart. A significant contribution was made by Alanis, Gonzailez, and L6pez in 1958 when they recorded His bundle potentials in the isolated perfused dog ...

His bundle ablation was performed in a 48 year old man with drug refractory orthodromic atrioventricular reentrant tachycardia and paroxysmal atrial fibrillation. Reentry was caused by a left free wall concealed accessory pathway. Interruption of the His bundle by low energy direct current shock (25 J) was quickly followed by anterograde conduction via the accessory pathway, with various forms ...
